Community Care Facilities (CCFs)

Every person who receives services through the San Andreas Regional Center (SARC) has a personalized plan called an Individual Program Plan, or IPP. This plan is like a roadmap—it helps set goals for the future and lists the support and services needed to reach them.


The IPP is created during a meeting with the individual, their family (if they choose), and their SARC Service Coordinator. It includes information about what's important to the person, what they want in life, and how SARC and others can help. The plan covers many areas like school, work, health, where someone lives, and how they spend their time.


The IPP changes as the person grows and their needs or goals change. It is reviewed regularly to make sure it still fits their life.

Lanterman Act Connection – Community Care Facilities (CCFs)

The Lanterman Act guarantees the right to safe, supportive living environments. Community Care Facilities provide 24-hour residential services tailored to varying levels of need, ensuring individuals with developmental disabilities receive care that respects their dignity and promotes community living.

Legal Foundation
  • W&I Code § 4681.1 – Outlines rates and service levels for Community Care Facilities, including Level 2 through Level 4I.

  • W&I Code § 4648(a)(6) – Authorizes regional centers to secure out-of-home services when in-home options do not meet the individual’s needs.

  • Health & Safety Code § 1500 et seq. – Establishes licensing requirements for Community Care Facilities operated by the California Department of Social Services.
